• Educates clients on self-help techniques and serves as a role model.
• Provides peer health navigation to connect clients with community services.
• Conducts outreach and linkage navigation to primary care, substance abuse treatment, and social supports.
• Accompanies clients to appointments and helps access resources.
• Documents progress notes and participates in team meetings and trainings.
• Assists in developing community support systems and housing transitions.
