Job summary

Haitian Centers Council Inc. is seeking a Patient Navigator to provide support, advocacy, and care coordination for individuals accessing mental health and supportive counseling services, with a focus on PLWHA.

You will work with clinical teams to ensure comprehensive care and linkages to services. The role emphasizes trauma-informed, culturally responsive support, housing assistance, and navigation of Medicaid, Ryan White programs, and related resources to reduce barriers and improve engagement

Qualifications

• Bachelor’s degree in Public Health, Psychology, Human Services, or related field preferred.
• Associate degree with at least 2 years of health navigation/case management/HIV/AIDS experience considered.
• Experience with PLWHA, LGBTQ+ communities, and mental health/substance use disorders is desired.
• Knowledge of harm reduction, trauma-informed care, and motivational interviewing preferred.
• Cultural competency with diverse populations (BIPOC, immigrant communities) is important.

Responsibilities

• Serve as a primary contact for clients seeking mental health and supportive counseling services.
• Conduct client intakes, needs assessments, and care plans.
• Develop and maintain up-to-date risk assessments and linkage to care.
• Provide education, emotional support, and advocacy for HIV/AIDS clients navigating behavioral health care.
• Assist clients in accessing mental health services, primary care, housing, and social resources.
• Create care plans addressing barriers to health outcomes and self-management.
• Collaborate with healthcare providers and community-based organizations.
• Attend community events to build partnerships and support warm hand-offs.
