Job summary

Via Care Community Health Center seeks an HIV Navigation Specialist to provide outreach, rapid HIV testing, care coordination, and case management for individuals at risk for or living with HIV. The role connects clients to PrEP/PEP and HIV medical care and supportive resources.

Collaborating with East LA Women’s Center and CHICAS program staff, the specialist promotes engagement in care, treatment adherence, and evidence-based interventions to improve health outcomes and reduce HIV transmission.

Qualifications

• Strong communication skills for outreach and education.
• Solid understanding of HIV, treatment, PrEP/PEP.
• Experience in care coordination and case management is preferred.

Responsibilities

• Conduct outreach and education with target populations in partnership with East LA Women’s Center Promotoras.
• Provide rapid HIV testing and confidential risk assessments.
• Coordinate access to PrEP/PEP and linkage to HIV medical care and support services.
• Assist with navigation around treatment adherence and partner services.
