Job summary

National Youth Advocate Program in Shelby County, Ohio, seeks a full-time Care Coordinator to support OhioRise. You will guide children and families through wraparound care coordination, working with providers, schools, and community partners to access needed services and build stronger support systems.

The role emphasizes collaboration, compassion, and advocacy, empowering families while coordinating care to improve health outcomes.

Qualifications

• Experience delivering community-based services to children and families.
• Knowledge of wraparound care coordination and multi-system needs.
• Ability to work collaboratively with families, providers, and community partners.

Responsibilities

• Develop and maintain wraparound care coordination plans.
• Coordinate and supervise implementation of the Plan of Care and Crisis Plan.
• Ensure family support and stabilization during crises.
• Maintain all service documentation and evaluation data.
• Participate in after-hours on-call rotation and CQI processes.
