Job summary

National Youth Advocate Program (NYAP) in Greene County, Ohio, seeks a full-time Care Coordinator to support the OhioRise program for children with complex needs. This role emphasizes wraparound care coordination, collaboration with families and providers, and guiding youths to access needed services within the High-Fidelity Wraparound model.

Qualifications

• Licensed or unlicensed practitioner under Ohio Administrative Code 5160-27-01.
• Complete the high-fidelity wraparound training program.
• Experience providing community-based services to children and families.
• Three years with a high school diploma or equivalent; or two years with an associate/bachelor; or one year with a Master’s degree or higher.

Responsibilities

• Deliver community-based wraparound care coordination.
• Develop and maintain the Wraparound Team; coordinate and lead meetings.
• Coordinate and supervise Plan of Care, Transition Plan, and Crisis Plan with providers.
• Ensure family support and stabilization during crises.
• Provide and document the Life Domain Assessment; maintain all required documentation.
• Participate in CQI peer review and Agency meetings; adhere to policies.
