Job summary

National Youth Advocate Program (NYAP) in Ohio is seeking a full-time Care Coordinator for the OhioRISE program in Shelby County. The role focuses on wraparound care coordination for children with complex needs, guiding families to access services and build supportive systems.

The position emphasizes collaboration, advocacy, and partnership with providers to ensure effective care coordination and improved health outcomes for youth.

Qualifications

• Must be a licensed or unlicensed practitioner per Ohio Administrative Code 5160-27-01.
• Complete High-Fidelity Wraparound training and ongoing skill development.
• Experience providing community-based services to children and families.

Responsibilities

• Cultivate flexible, family-focused, community-based services using Wrap Around model.
• Develop and lead the Wraparound Team and coordinate team meetings.
• Coordinate and supervise the Plan of Care and Transition/Crisis Plans with providers.
• Ensure family support during crises and document assessments.
• Maintain required service documentation and data reporting.
• Deliver timely services in accordance with the Plan of Care and Crisis Plan.
• Participate in after-hours on-call and CQI Peer review processes.
