Job summary

National Youth Advocate Program in Xenia, OH seeks an experienced OhioRise Moderate Care Coordinator to deliver community-based wraparound care coordination for youth with complex needs. You will guide families through OhioRise, coordinating services across behavioral health, education, and child welfare with partners and providers.

Qualifications

• High school diploma or equivalent; progressive education is preferred.
• Experience in community-based child and family services is required.
• Training in high-fidelity wraparound is required or to be completed.

Responsibilities

• Coordinate and lead the Wraparound Team meetings.
• Develop and maintain the Wraparound Team and Plan of Care.
• Oversee and implement Transition and Crisis Plans with providers.
• Ensure family support and stabilization during crises.
• Document Life Domain Assessments and maintain required data.
