Job summary

National Youth Advocate Program seeks a full-time OhioRise Care Coordinator in Wapakoneta, OH to support children with complex needs through wraparound care coordination. The role emphasizes collaboration, advocacy, and empowering families while coordinating with providers to access needed services.

The position requires experience in behavioral health and related fields, living in the Catchment Area C region, and a commitment to building networks across counties.

Qualifications

• Licensed or unlicensed practitioner per rule 5160-27-01 of the Administrative Code.
• Complete High-Fidelity Wraparound training program.
• Complete skill and competency-based training for MCC.
• Experience providing community-based services to children and youth and families in behavioral health, child welfare, etc.
• Live in Catchment Area C (Allen, Auglaize, Champaign, Clark, Darke, Hardin, Greene, Logan, Madison, Miami, Shelby)

Responsibilities

• Cultivate flexible, family-focused, community-based responsive services using the High Fidelity Wrap Around model.
• Develop and maintain the Wraparound Team and lead team meetings.
• Coordinate and supervise Plan of Care implementation, Transition Plan, and Crisis Plan with providers.
• Ensure family support and stabilization during crises.
• Provide Life Domain Assessments and document services.
• Maintain all service documentation and data.
• Provide timely services per Plan of Care and Crisis Plan.
• Participate in after-hours on-call and CQI processes.
• Attend staff meetings and trainings as required.
• Report MUls to Site Manager promptly.
• Other duties as assigned.
