National Youth Advocate Program (NYAP) seeks a full-time CME Moderate Care Coordinator to join our OhioRise team in Auglaize County, Ohio. The position focuses on wraparound care coordination for children with complex behavioral health and multi-system needs under ODM's OhioRise program.

With a salary of $47,000 per year, NYAP emphasizes family-centered services, collaboration with providers, and ongoing professional growth through CEUs, tuition reimbursement, and comprehensive benefits.

Qualifications

• Experience providing MCC or similar wraparound care coordination for youth and families.
• Knowledge of OhioRISE program and multi-system involvement.
• Ability to develop and implement Plan of Care and Transition Plan.

Responsibilities

• Cultivate Wraparound services based on High Fidelity model.
• Lead and coordinate the Wraparound Team and meetings.
• Oversee Plan of Care and Crisis Plan with providers and community resources.
• Ensure family support and stabilization during crises.
• Document Life Domain Assessments and maintain required records.
• Adhere to agency policies and regulatory standards.
