Job summary

National Youth Advocate Program (NYAP) is hiring a Care Coordinator for the OhioRise program in Shelby County, OH. The role delivers community-based wraparound coordination for children with complex needs, guiding families through the OhioRise process and coordinating with providers and community resources to improve outcomes.

Under supervision, you will support families to stay safely at home and in their communities, while reducing out-of-home placements.

Qualifications

• Experience providing community-based services to children and families.
• Background in one or more areas: family systems, community resources, case management, counseling, child protection, or child development.
• Must meet educational pathways: HS diploma or higher, with higher degrees offering options.

Responsibilities

• Cultivate flexible, family-focused, community-based services based on Wrap Around model.
• Develop and maintain the Wraparound Team and lead meetings.
• Coordinate and supervise the Plan of Care, Transition Plan and Crisis Plan.
• Ensure family support during crises and document life domain assessments.
• Maintain all required service documentation and data.
• Provide timely services per Plan of Care and Crisis Plan.
