Job summary

ACH Child and Family Services in Fort Worth, TX seeks a Clinical Case Manager-Treatment Services to support youth and families by coordinating case management, medical and behavioral health services, and crisis management within a trauma-informed framework. You will develop assessments and service plans, coordinate resources to stabilize behaviors, and ensure timely documentation.

The role requires strong communication and collaboration with families, guardians, and program staff to deliver

Qualifications

• Bachelor’s degree in social work, psychology, or related human services field is required.
• Master’s degree and/or additional certification is preferred.
• Experience with adolescents and families in crisis intervention is strongly preferred
• Excellent verbal and written communication skills
• Working knowledge of basic computer skills

Responsibilities

• Provide and coordinate case management, medical and behavioral health services.
• Crisis management, aftercare planning and transitional services.
• Maintain client files and ensure documentation meets standards.
• Coordinate services with guardian, CASA, caseworkers and partners.
• Complete assessments, service plans and progress notes.
