Job summary

Cumberland Youth and Family Services seeks a Foster Care and Adoption Case Manager to manage caseloads for foster children and homes, coordinate with other agencies, and advocate for youth rights and educational needs.

Responsibilities include coordinating with STAR Health, HHSC Behavioral Health Services, and education partners, and maintaining detailed case documentation. This hybrid Texas position requires relevant degrees and experience.

Qualifications

• Master's degree in social work from an accredited college or university.
• One year of documented full-time work experience in a child placing agency, in a residential child-care operation, or as a conservatorship caseworker or foster adoptive home development worker for the department.
• The experience must include conducting assessments, service planning and case management duties.
• The experience may include a maximum of 350 hours of formal, supervised field placement or practicum in child placement activities.
• Bachelor’s degree from an accredited college or university and
• Two years of documented full-time work experience in a child placing agency, in a residential child-care operation, or as a conservatorship caseworker or foster adoptive home development worker for the department.
• Two years working under the direct supervision of child placement management staff.
• Working knowledge of Minimum Standards for Child Placing Agency
• Must complete background checks required by Texas Department of Family & Protective Services (DFPS) Central Registry, Texas Department of Public Safety, national fingerprint registry, and drug/alcohol testing
• A valid Texas Driver’s License

Responsibilities

• Utilize evidence-informed TBRI trauma-informed care for children who have been victims of abuse/neglect.
• Ensure regular individual and family therapy through referrals and monitoring.
• Facilitate service planning team meetings.
• Ensure CANS 3.0 informs service delivery and appropriate services.
• Advocate for youth rights and goals.
• Coordinate school enrollment and accommodations.
• Coordinate care with STAR Health and HHSC services.
• Engage with family and kin-centered support networks throughout placement.
• Respond to crises during on-call rotations.
• Create Aftercare Services plan with ongoing follow-ups for six months.
