Diamond Experience Co. is seeking a Qualified Mental Health Professional/Case Manager to deliver Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families in Texas.

The role requires a Bachelor's degree in a related field and a minimum of two years in behavioral health, plus CANS/ANSA certification. The incumbent will collaborate with clinical teams, maintain thorough documentation in the EHR, and ensure compliance with Medicaid

Qualifications

Bachelor's degree in social work, psychology, or related field is required.
CANS/ANSA certification is required.
At least two years of experience in behavioral health or related field.
Must have reliable transportation, valid driver's license, insurance, and clean driving record.
Must have access to computer and internet; proficient with MS Word/Excel/PowerPoint and EHR.

Responsibilities

Adhere to Medicaid/Insurance guidelines.
Develop service/treatment plans focusing on child and family needs.
Provide skill-building training in home or community settings using evidence-based practices.
Offer crisis/emergency support via face-to-face, phone, or virtual contact.
Maintain client documentation in the EHR and write progress notes per Texas ADC requirements.
Enter data clearly and timely; complete daily session notes within 24 hours.
Document non-billable contacts to show follow-up history.
Support tracking of outcomes; assist in record reviews as needed.
Use active listening to build rapport with clients and families; provide encouragement.
