LSS of South Dakota is seeking a dedicated Case Manager to support youth as they move toward independence. You will develop individualized plans, conduct assessments, and connect clients with community resources to foster growth and positive change.

The role requires a bachelor's degree in social work or a related field, eligibility for SD social work licensure within six months, and strong organizational and communication skills. A valid driver's license and auto-insurance are required.

Qualifications

• Bachelor's degree in social work or a related field is required.
• Must be at least 21 years old.
• Licensure as SW or SW Associate through SD Board preferred; licensure within 6 months.

Responsabilités

• Develop individualized case plans and conduct ongoing assessments.
• Connect youth with community resources and support their growth.
• Maintain accurate case documentation; coordinate medical, dental and vision appointments; transport as needed.
