Job summary

Resilient Youth and Community is seeking a Case Manager to support adolescents in residential treatment programs and their families. You will coordinate services, document care, and liaise with the treatment team to ensure cohesive planning and effective linkage to supports.

The role requires strong written and verbal communication, organization, and the ability to work both in teams and independently. Monday–Friday, 8-hour shifts, with comprehensive benefits and a focus on youth well-being.

Qualifications

• Excellent written and verbal communication skills.
• Exceptional organizational skills and ability to multi-task in a fast-paced environment.
• Ability to work well as a team and independently as duties require.
• Ability to implement coping strategies and conflict resolution in potentially volatile situations.

Responsibilities

• Provide case management services to clients and families, coordinating treatment & services and scheduling appointments.
• Develop treatment plans with clinicians and clients.
• Maintain and update all required paperwork and documentation.
• Maintain internal and external databases through regular data entry and updates.
• Attend and participate in agency meetings and communicate relevant information.
• Comply with regulations and agency policies to meet licensing and accreditation standards.
• Maintain confidentiality and professional ethics.
• Engage with teens to facilitate participation in RYC services.
• Participate in trainings to build skills for working with youth and families.
• Other duties as assigned.
