Qualifications

• 2-4 years of experience in Case Management or behavioral health setting preferred.
• 1-2 years of experience in leading teams preferred.
• Must have a valid driver’s license.

Responsibilities

• Provide wraparound services to families with a limit of up to 15 members/families.
• Coordinate a comprehensive set of supports for each member and family.
• Facilitate team meetings with youth, family, and team members.
