Job summary

Outreach is hiring a Full-Time Case Manager at Outreach Carol Stream to provide case management services to individuals and families in crisis or seeking stability. You will assess needs, advocate, coordinate resources, and manage emergency funds with reporting and compliance.

Requirements include a Bachelor's degree in social work or related field, at least 2 years of trauma-informed experience, proficiency with Word/Excel, and a valid Illinois driver's license. Bilingual skills preferred.

• At least 2 years of trauma-informed direct services to low-income families.
• Previous case management experience.
• Proficiency in Microsoft Word and Excel.
• Bilingual proficiency preferred.
• Illinois driver's license and reliable vehicle.

Responsibilities

• Assess client needs, provide case management and advocacy, connect individuals and families to community resources and referrals.
• Manage emergency assistance funds from local and federal funding sources, determine eligibility, distribute funds according to program guidelines, and maintain accurate financial records and reports.
• Maintain a comprehensive directory of community resources and develop referral relationships to support client needs.
• Collaborate with Outreach Carol Stream's counseling and youth development programs to provide case management and wraparound support.
• Participate in community outreach initiatives, provider networking groups, and the planning and coordination of Outreach Carol Stream events.
• Maintain accurate, timely, and confidential client records in accordance with Outreach's data standards and reporting requirements.
• Collaborate with case management staff across Outreach locations and participate in staff meetings, supervision, and organizational training.
• Schedule: Monday – Friday, 10:00 am to 6:30 pm. Occasional evenings to meet client needs.
