Qualifications

• 1+ year of case management experience in a social service or healthcare setting.
• Bachelor's degree in psychology, social work, or related field from an accredited college or university.

Responsibilities

• Conducts thorough assessment of clients and evaluates problem and service needs.
• Develops a written client service plan specifying goals to be achieved and services to be pursued.
• Provides, as appropriate, supportive help, advocacy and case management to individuals and families in coping with environment and interpersonal relationship issues.
• Works with community resources and develops knowledge of resources to assist clients.
• Collaborates with other program staff in planning and implementation of service plans.
• Handles emergency and crisis situations within target population with supervisory approval.
