Job summary

A community-focused service organization is seeking a Case Manager/Wraparound Facilitator to provide essential wraparound services to families. The ideal candidate will manage up to 15 families and coordinate comprehensive supports and resources. This full-time position requires a Bachelor's degree in a human service field and experience in behavioral health settings. A competitive salary of approximately $47,500 per year and benefits including health insurance are offered.

Qualifications

• 2-4 years of experience in Case Management and/or Outpatient/Inpatient behavioral health setting preferred.
• 1-2 years of experience in leading teams preferred.
• Must have a valid driver’s license.

Responsibilities

• Provide wraparound services to families with a limit of up to 15 members/families.
• Coordinate supports, resources, and strategies for each member and family.
• Utilize a strengths-based approach to facilitate team meetings.
