Qualifications

• 2-4 years of experience in Case Management or behavioral health setting preferred.
• 1-2 years of experience in leading teams preferred.
• Must have a valid driver’s license and provide three professional references.

Responsibilities

• Provide wraparound services to families with up to 15 members.
• Coordinate supports and resources for each family member.
• Facilitate team meetings with families and team members.
