Qualifications

• 2-4 years of experience in Case Management or behavioral health setting preferred.
• Must have valid driver’s license and reliable transportation.
• Experience with virtual platforms is a plus.

Responsibilities

• Provide wraparound services to families, coordinating supports and resources.
• Utilize strengths-based approach for planning and facilitating meetings.
• Administer assessments and monitor implementation of service plans.
