Lakes-Regional-Community-Center is seeking a Wraparound Coordinator to lead and coordinate planning for children, youth, and families with complex needs. You will facilitate family-centered team meetings and ensure services are organized collaboratively and strengths-based.

The role emphasizes honoring the family as the primary partner, ensuring their voice guides decisions, and supporting stability at home, in school, and in the community.

Qualifications

• Requires a Bachelor’s degree in Health Science, Social Work, Psychology or related field.
• Valid driver’s license and acceptable driving record required.
• Personal automobile liability insurance as required by the State of Texas.
• Preferred: 1 year serving youth with severe emotional disturbance.

Responsibilities

• Lead and coordinate the Wraparound planning process for children, youth, and families with complex needs.
• Facilitate structured, family-centered team meetings.
• Ensure services and supports are organized in a collaborative, strengths-based manner.
• Honor the family as the primary partner and ensure their voice guides planning and decision-making.
• Support families in achieving stability in homes, schools, and communities.
