Job summary

Excelsior Wellness Center is seeking a Care Coordinator to join a multidisciplinary team and engage families toward therapeutic goals. You will coordinate cross-system care plans, advocate for family needs, and help maintain family unity while navigating community resources.

The role emphasizes collaboration with clinicians, peer specialists, and education professionals to support youth and families and improve outcomes across systems.

Qualifications

• High School Diploma required; BA in counseling or related field preferred.
• Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
• Comprehensive knowledge of community resources and public systems.
• Minimum three years of working with youth and families.
• Community Health Worker certification preferred.

Responsibilities

• Provide direction in development and implementation of cross-system care plans to help youth and families remain safely at home.
• Advocate for families to preserve family unity and improve safety and satisfaction.
• Coordinate cross-agency communication and planning as needed.
• Provide transportation support and accompany students/families to activities, school, court and visits.
