Job summary

Northern Lakes Community Mental Health is seeking an Intensive Care Coordination with Wraparound specialist to support children with severe emotional disturbance or developmental disabilities. You will assess, plan, and coordinate services across family and community systems, in office or home settings, working with MDHHS Wraparound requirements.

Qualifications

• Bachelor's degree in human services or nursing-related credentials.
• One year experience providing direct services to children and families.
• Ability to be credentialed by NLCMHA to provide services in this discipline.

Responsibilities

• Provide direct clinical services to registered consumers, primarily children and families.
• Demonstrate knowledge of developmental disabilities and person-centered practices.
• Coordinate Wraparound planning, team facilitation, and service coordination.
• Engage families, crisis intervention, and transition planning.
• Facilitate Child and Family Team meetings with family voice and choice.
• Document Wraparound processes, engagement, planning, and monitoring.
• Identify natural supports and community resources for long-term stability.
• Develop transition plans to reduce reliance on formal services.

Education
