Job summary

Connected Families NH is seeking a Care Coordinator to provide family-centered planning facilitation and develop plans of care for children, youth, and families. The role involves coordinating services with multiple agencies, engaging families, and ensuring outcomes align with home, school, and community goals.

The coordinator will work in homes and communities, emphasizing family strengths, cultural competence, and high fidelity to the NH Wraparound Model. Travel within the region is expected.

Qualifications

• Bachelor’s degree in related field required.
• Minimum 2 years’ experience in health care, social services, behavioral health care, social work, human services, or education with care management experience.
• High school diploma or equivalency with experience in care settings and required training/certification for the care coordinator.

Responsibilities

• Maintains a caseload of 12 in weekly face-to-face wraparound planning and team meetings.
• Engage family in New Hampshire Wraparound process to identify unmet needs and supports.
• Develops and supports a family-based Plan of Care per NH Wraparound Manual.
• Coordinates and monitors services between agencies to meet identified needs.
• Identifies natural supports and community opportunities to build strengths and resilience.
• Establishes trusting relationships with child/youth clients and families.
• Schedules and facilitates family team wraparound meetings.
• Adheres to NH Wraparound Model and values.
• Enters case information into the data system.
• Follows ethical reporting and confidentiality standards.
