Job summary

Northern Lakes Community Mental Health is seeking an Intensive Care Coordination with Wraparound (ICCW) clinician to support children with SED or IDD. The role involves direct assessment, service provision, and coordination with family and community partners in multiple locations including Cadillac, Grayling, or Houghton Lake.

The position requires a degree in Human Services or Nursing, Michigan licensure as LBSW/LLBSW preferred, with additional credentials to be obtained as needed.

Qualifications

• Bachelor's Degree in Human Services or Nursing.
• Licensed with Michigan as specified (LBSW/LLBSW preferred; RN or non-licensed acceptable).
• One year experience in direct services to children and families.

Responsibilities

• Provide direct clinical services to children and families, ~30% of week.
• Coordinate Wraparound care planning and team facilitation.
• Maintain program statistics and required MDHHS documentation.
• Collaborate with community systems and schools for integrated care.
• Complete training requirements and quality improvement activities.
