Job summary

Northern Lakes Community Mental Health is hiring an Intensive Care Coordination with Wraparound (ICCW) specialist to support children with severe emotional disturbances or developmental disabilities. The role combines direct clinical work, Wraparound planning, and cross-system collaboration in office, home, or community settings.

The position requires a Bachelor's in Human Services or Nursing licensure and state credentialing with NLCMHA.

Qualifications

• Bachelor's Degree in Human Services or Nursing licensure is required.
• Credentialing by NLCMHA to provide services in this discipline is necessary.
• One year of direct services experience with children and families is preferred.

Responsibilities

• Provide direct clinical services to children and families, ~30% of the workweek.
• Coordinate Wraparound planning, team facilitation, and service coordination.
• Engage with families, document interventions, and monitor outcomes and progress.
• Attend team meetings and collaborate with schools, child welfare, and other agencies.
