Job summary

Lutheran Social Services of SD is seeking a Wraparound Care Coordinator to implement the CARES model, coordinating services for children, youth and families. The role centers families’ voices, strengths, and choices to reach goals and connect resources.

You will work with families, community partners, and referral sources to create plans of care, coordinate services, and monitor progress. Some travel is required.

Qualifications

• Bachelor’s degree in social work or related field required or eligible.
• Licensure as a Social Worker or Social Work Associate preferred.
• Must possess or be eligible for licensure through the SD Board of Social Work Examiners.

Responsibilities

• Provide strengths-based, family-driven case management and Wraparound services for children, youth, and families.
• Facilitate Family Team Conferencing and Wraparound process with teams and partners.
• Develop individualized Plans of Care and Crisis Plans; coordinate services and supports.
• Maintain accurate documentation in required systems and complete program data.
