Lutheran Social Services of SD is seeking a Wraparound Care Coordinator to implement the CARES model, coordinating services for children, youth and families. The role centers families’ voices, strengths, and choices to reach goals and connect resources.
You will work with families, community partners, and referral sources to create plans of care, coordinate services, and monitor progress. Some travel is required.

HOURS: Full-Time, 40 hours per week, occasional evenings as needed.
SALARY: $21.00-$23.00 per hour, non-exempt. Dependent on experience and qualifications.
TRAVEL: Travel is required. Mileage reimbursement.

As a Care Coordinator, you will implement the C.A.R.E.S. Wraparound model by providing strengths-based, family-driven case management and coordination of services for children, youth, and their families. The C.A.R.E.S. Wraparound model is a child abuse prevention and diversion program designed to support families early in the child welfare continuum and help them build upon their strengths, resources, and natural supports. You will work directly with families to bring the appropriate people and supports together, help families identify their strengths and needs, develop a plan, connect them with resources, and stay involved to help the plan succeed.
You will facilitate Family Team Conferencing and the Wraparound process, working collaboratively with families, youth, community partners, and referral sources to identify strengths and needs, develop individualized Plans of Care, and coordinate appropriate services and supports. You will promote a family-driven, youth-guided, community-based, culturally responsive, and trauma-informed approach.
QUALIFICATIONS: Candidates must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners. Obtaining and maintaining licensure is a requirement of this position. Guidance through the licensure process will be provided by Lutheran Social Services of SD.
