Wisconsin Community Services in Milwaukee is seeking a dedicated Care Coordinator for the Wraparound Milwaukee program. This role focuses on intensive, individualized care coordination for children and adolescents within a trauma-informed, strength-based framework.

You will manage a caseload of about 8 families, run weekly visits, facilitate Child and Family Team Meetings, and develop the initial Plan of Care within 30 days, including a 24-hour Crisis Plan.

Competenze

• Bachelor’s degree in a relevant field and at least 1 year of experience in mental health services.
• Complete 85 hours of Wraparound training and attend Family Orientation within 6 months.
• Proficient with Windows and Microsoft Word.
• Demonstrates cultural competence with diverse populations.
• Experience working with children, youths and families.
• Valid driver’s license, automobile, and insurance per agency requirements.
• Clear background checks and drug testing; meets employment requirements.

Mansioni

• Maintain a caseload of approximately 8 children and families.
• Schedule and attend weekly visits and facilitate monthly Child and Family Team Meetings.
• Assemble a Child and Family Team within two weeks of enrollment.
• Within 30 days, coordinate the initial Plan of Care including a comprehensive 24-hour Crisis Plan.
• Monitor the provision and quality of services and liaise for new resources.
• Advocate for children and families across home, school, court and community settings.
• Attend scheduled court hearings representing Wraparound Milwaukee.
• Arrange transportation and connect families to resources as needed.
• Complete required paperwork per agency guidelines.
• Keep family demographic information up to date and collaborate with involved parties.
