Job summary

Lutheran Social Services of SD is seeking a Wraparound Program Manager to lead the Wraparound/Family Stabilization team. You will guide families through care planning, coordinate resources, and support crisis planning to promote child and family well-being.

You will work with staff, co-facilitate Family Team Conferences, and ensure effective program reporting and budgeting. Strong collaboration with community partners is essential.

Qualifications

• Bachelor’s degree in social work or related field is required + four years related experience.
• Licensure as Social Worker or Social Work Associate is preferred.
• Must be at least 21 years of age and able to pass background checks.
• Active auto-insurance and valid driver’s license or ability to obtain one.

Responsibilities

• Provide information and intake services to families, referral sources, and community partners regarding Wraparound services and the C.A.R.E.S. model.
• Facilitate Family Team weekly meetings and guide families through the Wraparound process, including Plans of Care and Crisis Plans.
• Assess family strengths, needs, resources, and circumstances for service planning.
• Provide counseling, resource coordination, and referrals to support families.
• Maintain professional relationships with families and partners; coordinate services.
