• Bachelor's degree in social work, psychology, counseling, or related field.
• 2+ years experience in case management, care coordination, social services, or mental health settings; wraparound experience a plus.
• Strong communication, organizational and time-management skills.

Responsibilities

• Engage with clients and families to establish trust and assess needs.
• Develop and manage individualized care plans addressing mental health, education, physical health, and social needs.
• Coordinate with wraparound team members and community agencies to implement care plans.
• Advocate for clients and families across service systems and facilitate team meetings to review goals.
• Monitor progress, document plans and outcomes, and adjust interventions as needed.
