Job summary

National Youth Advocate Program (NYAP) seeks a full-time Care Coordinator for the OhioRise team serving Logan, Champaign, and Shelby counties in Ohio. The role focuses on wraparound care coordination for youth with complex needs, guiding families through services to improve outcomes and sustain placements in home and community settings.

This position emphasizes collaboration, advocacy, and partnership with providers.

Qualifications

• Three years with a high school diploma or equivalent, or two years with an associate’s or bachelor’s degree, or one year with a Master’s or higher.
• Experience providing community-based services to children and families in behavioral health, child welfare, ID/DD, or related public sector fields.
• Licensed or unlicensed practitioner per Ohio administrative rule 5160-27-01 and completion of High-Fidelity Wrap Around training.

Responsibilities

• Cultivate flexible, family-focused, community-based wraparound services.
• Develop and maintain the Wraparound Team and lead meetings.
• Coordinate and supervise implementation of the Plan of Care and Transition/Crisis Plans.
• Ensure family support during crises and document assessments.
• Maintain required service documentation and evaluation data.
• Provide timely services in accordance with the Plan of Care and Crisis Plan.
• Utilize Flexible Funding and monitor service coordination.
• Obtain weekly reports from subcontracted providers.
• Participate in after-hours on-call and program CQI reviews.
• Attend staff meetings and comply with agency policies.
