National Youth Advocate Program (NYAP) in Lima, Ohio seeks a full-time CME Care Coordinator Supervisor to lead a team delivering wraparound care coordination for youths with complex needs. The role focuses on family-centered, community-based services and reducing out-of-home placements.

Responsibilities include recruiting and training staff, achieving program goals, and ensuring high-quality documentation and collaboration with stakeholders.

Qualifications

• Experience supervising care coordination staff in behavioral health or related field.
• Background in wraparound service delivery or similar models.
• Ability to manage teams and ensure program goals are met.
• Familiarity with OhioRISE or multi-system youth services preferred.

Responsibilities

• Recruit, interview and train Care Coordinators.
• Achieve monthly and yearly goals for staff and program.
• Provide formal and informal staff training and development.
• Conduct evaluations and develop action plans for staff growth.
• Provide regular supervision and feedback to assigned staff.
