National Youth Advocate Program is seeking a full-time CME Care Coordinator Supervisor in Sidney, OH to lead a team delivering wraparound care coordination for youth with complex needs. You will supervise coordinators, ensure quality service plans, and collaborate with families and communities to keep youth safe at home and in school.

The role requires a 4-year degree, strong communication and supervisory skills, and experience with community-based behavioral health services.

Qualifications

• Requires a 4 year degree or higher.
• Experience providing community-based services to children and families in behavioral health or related fields.
• Strong written and verbal communication; ability to supervise and train staff.

Responsibilities

• Lead a team of Care Coordinators to deliver wraparound services.
• Recruit, interview and train new staff; monitor performance and development.
• Ensure monthly and yearly goals are met and maintain documentation quality.
• Provide regular supervision and feedback; coordinate meetings and case planning.
