Job summary

The Buckeye Ranch in Columbus, OH is seeking a Care Coordination Supervisor to lead our Care Management Entity (CME). You will supervise care coordination teams, ensure adherence to the high fidelity wraparound model, and keep services child, youth, and family-centered with strong outcomes.

Responsibilities include supervising staff, coordinating with schools and providers, guiding training plans, and monitoring program budgets to ensure quality services and family voice.

Qualifications

• Bachelor's degree in human services; master's degree preferred.
• Two years of relevant working experience.
• Active LSW or LPC preferred.
• Valid driver's license and proof of auto insurance.

Responsibilities

• Ensure voice and choice in coordinated care for care coordinators.
• Develop and implement training plans for care coordinators with schools and providers.
• Provide supervision for direct report care coordination team members.
• Conduct group supervision and team meetings.
• Collaborate with Utilization Manager on case assignment and production.
• Monitor program budgets.
• Collaborate with systems to ensure high-quality services with family voice and choice.
