Integrated Services for Behavioral Health (ISBH) seeks a Care Coordination Supervisor to oversee a team delivering high-fidelity wraparound care coordination for children and families in Ohio. The role emphasizes staff training, documentation, and collaboration with community partners.

The ideal candidate will have a social work/related degree, supervisory experience, and strong knowledge of behavioral health regulations. A $5,000 hiring bonus is available and Medicaid standards are emphasized.

Qualifications

• Bachelor’s or master’s degree in social work, education, counseling, psychology, or related field.
• LSW/LPC preferred.
• Three years of experience in care coordination.
• Supervisory experience preferred.
• Experience in behavioral health field required.
• Knowledge of regulations for behavioral health and autism.
• Excellent communication and organizational skills.
• Experience with Electronic Health Records (EHR).
• Ability to conduct video calls/meetings in an automated office.
• Requires high-speed internet (minimum 100 Mbps).

Responsibilities

• Supervise and oversee care coordination team activities.
• Provide training for direct care coordinators in home and community settings.
• Support staff through conferences and case problem analysis.
• Promote teamwork and recognition among staff.
• Maintain high fidelity wraparound training credentials.
• Ensure completion of care plans, assessments, and reports.
• Coordinate on-call rotation and fidelity reviews.
• Collaborate with systems and partners to ensure family voice and choice.
• Assess needs in crisis situations and inform response.
• Maintain confidentiality and ensure Medicaid standards.
