Job summary
Hartford HealthCare at Home seeks a Wound Program Specialist to develop and coordinate agency-wide wound prevention and management programs. The role provides expert consultation and hands-on care for patients with acute or chronic wounds, and coordinates multi-disciplinary care plans with staff and community partners.
The ideal candidate holds a Bachelor’s in nursing, RN license, WOCN certification (MSN preferred), and 2+ years in clinical specialty/homecare.
Qualifications
- Bachelor’s Degree in nursing is required.
- WOCN certification is required.
- MSN degree preferred.
- RN licensure required and Wound/Ostomy certification must be obtained within 6 months of hire.
Responsibilities
- Develops and coordinates agency-wide wound prevention and management programs.
- Provides expert consultation and hands-on wound care for patients.
- Coordinates multi-disciplinary, comprehensive management plans and care for patient populations.
- Develops educational programs for patients, staff and other providers.
- Coordinates quality improvement initiatives to optimize wound outcomes.
- Advocates for patients with skin and wound care needs.
- Reviews regional wound program metrics for performance.
