The Alden Network in Illinois seeks a Wound Care Coordinator (RN) to oversee our wound care program for residents at Alden Poplar Creek. The role requires daily rounds, development of care plans, and coordination with CNAs, nurses, physicians, and therapists to ensure proper prevention and treatment protocols are followed.

The RN must lead wound-related meetings, document in risk management systems, and participate in a multidisciplinary team to optimize healing and prevent new wounds.

Competenze

• Must be a licensed RN in Illinois.
• Demonstrates strong communication with residents, families and staff.
• Ability to work with interdisciplinary team to optimize care.

• Conduct daily rounds to assess wound care program adherence.
