Job summary

The Alden Network is seeking a Wound Care Coordinator (RN) in Illinois to lead the skin care program for residents at Alden Poplar Creek. The role involves prevention, wound treatment protocols, and collaboration with the Interdisciplinary Team to optimize patient care.

The WCC educates CNAs and nurses, participates in care plan development, and ensures compliance with policies and procedures. Strong communication and physical ability for daily rounds are required.

Qualifications

Licensed RN in Illinois.
Experience with wound care preferred.
Ability to lift 35 pounds.
Strong communication and customer service skills.
Willingness to participate in ongoing training.

Responsibilities

Oversee wound care program for residents in the facility.
Educate CNAs and nurses on prevention and treatment protocols.
Collaborate with Interdisciplinary Team to optimize care.
Conduct daily rounds to assess turning/repositioning compliance.
Develop interventions and essential care plans.
Document in risk management system and write timely assessments.
Lead Weight & Wound meetings with nursing staff and managers.
