Job summary

The Alden Network in Long Grove, IL seeks an experienced RN or LPN to lead wound care services for residents with ostomies, draining wounds, fistulae, stomas, and soft tissue injuries while supervising the nursing staff in day-to-day wound care activities.

The role emphasizes adherence to policies, wound assessments, patient education, and collaboration with physicians and allied health professionals; CPR is required.

Qualifications

• Must possess a current, unencumbered license to practice as an RN or LPN in Illinois.
• Current CPR certificate or be certified within two weeks of employment.
• Knowledge of nursing and medical practices and procedures.
• Demonstrates interest in care of the aged.
• Must be able to read, write, and speak English clearly.
• Ability to make independent decisions when required.
• Ability to deal tactfully with customers, family, personnel and agencies.
• Leadership and supervisory ability; teamwork with staff.
• personable, enthusiastic, and resourceful attitude.

Responsibilities

• Must ensure that all nursing procedures and protocols are followed per policies.
• Directly supervise nurses, CNAs, and other IDT members in wound care.
• Oversee wound technicians and wound care protocols.
• Assess and document wound status and skin care.
• Administer or assist with wound treatments as ordered by physician.
• Review treatment orders for completeness and accuracy.
• Review wound assessments for completeness and accuracy.
• Complete weekly skin reports and send to nurse consultant and pharmacy.
• Educate facility staff on pressure ulcer prevention.
• Coordinate diagnostic and therapeutic services for wound care.
• Assist in obtaining wound specimens as ordered.
• Update family and physician on wound changes.
• Develop written care plans and evaluate delivery of care.
• Participate in care plan conferences.
• Consult with nurses, management and health professionals on care delivery.
• Plan and participate in in-service education programs.
• Make written and oral reports with recommendations for wound care.
• Ensure care plans are followed and wound assessments reflect this.
• Participate in QA program for wound care.
• Coordinate wound audits with nursing staff.
• Make daily rounds to ensure duties and protocols are followed.
• Communicate wound progress on 24-hour report.
• Commit to a clean and safe environment for residents.
• Complete forms during admission, transfer and discharge.
• Attend staff meetings and perform other duties as assigned.
