The Alden Network in Illinois seeks a Wound Care Coordinator (RN) to oversee our wound care program for residents at Alden Poplar Creek. The role requires daily rounds, development of care plans, and coordination with CNAs, nurses, physicians, and therapists to ensure proper prevention and treatment protocols are followed.

The RN must lead wound-related meetings, document in risk management systems, and participate in a multidisciplinary team to optimize healing and prevent new wounds.

Job description

JOIN OUR TEAM!

The Wound Care Coordinator (WCC) must be a licensed RN in the state of Illinois. This nurse is responsible for the overall skin care program for all residents who reside at Alden Poplar Creek. This includes prevention techniques as well as treatment protocols for wounds. The Wound Care Coordinator RN educates and works directly with CNA's and Nurses to ensure that treatment and prevention policies/protocols are followed according to our established policies and procedures. Collaboration with the Interdisciplinary Team including the physician, nurse practitioner, and other diagnostic and therapeutic services to optimize care is an essential part of the job and requires the applicant to be professional and possess excellent communication skills.

The Wound Care RN must have the drive, fortitude, and physical ability to make daily walking rounds throughout the facility to assess compliance with the turning and repositioning program. The development of interventions, creation of essential care plans, documentation in a risk management environment, and written and timely assessments, are all functions of the position. The Wound Care RN will direct the Weight & Wound meetings and actively participate with other nurses, managers, and related health professionals to coordinate a plan of care directed to healing existing wounds and prevention of any new development of wounds.

This Wound Care RN is a part of the overall clinical management team and assists in the leadership and direction of the nursing department. The Wound Care RN reports directly to the Director of Nursing. Candidates must be team players, energetic, resourceful, able to work independently, fun spirited, willing to participate in change where needed, and possess excellent customer service skills.

This facility has approximately 150 residents and caters to the long term care population. Salary is commensurate with licensure type and experience . We will work with individuals to train them for this opportunity, but would prefer some level of wound care experience. Candidate must be able to lift 35 pounds and carry it 25 feet, have a good command of the English language for both written and verbal communication, be computer literate, and pass both a drug test and healthcare worker's background check.
