Alden Poplar Creek Rehabilitation and Health Care Center is seeking a Wound Care Coordinator RN to manage the wound care program for all residents at the facility. The role involves implementing prevention techniques and treatment protocols, educating CNAs and nurses, and collaborating with the interdisciplinary team to optimize resident care.

The RN must be able to conduct daily rounds, develop care plans, document assessments, and participate in Weight & Wound meetings.

Qualifikationen

• Licensed RN in Illinois.
• Wound care experience preferred.
• Ability to educate CNAs and nurses on policies.
