Job summary

Fairview Health Services is seeking a Care Coordinator RN to join our team at the Clinics and Surgery Center in Minneapolis, MN, primarily at the Maple Grove Specialty Clinic. This full-time 1.0 FTE role runs Monday through Friday, with 8-hour shifts from 8:00 a.m.

Qualifications

• RN licensure and BSN or equivalent four-year degree required.
• Certificate: Basic Life Support (BLS) required or as specified; alt. exception for certain dept 8901.
• Experience in wound care and ambulatory care coordination is preferred.

Responsibilities

• Coordinate patient-centered care across the continuum.
• Provide direct wound care and education to patients and families.
• Facilitate referrals and follow-up care with providers and community partners.
• Document patient progress and evaluate nursing care quality.
