Advocate Health is seeking a Wound Care Coordinator RN to oversee wound/skin management within the Nursing Quality and Practice department. You will lead comprehensive assessments, develop care plans, and educate patients and families to support healing and prevention of complications.

The role requires BSN, RN licensure, board certification by 2015, and 4 years of wound care experience. 40-hour weeks, full-time, on-site with possible rotating weekends and no holidays.

Qualifikationen

• Bachelor of Science in Nursing (BSN) required.
• Board Certification by 12/31/2015 or within two years of hire.
• 4 years clinical RN experience in wound care.

• Respond to consultation requests in a timely manner.
• Provide direct wound care to selected patients.
• Perform comprehensive patient and family assessments.
• Develop comprehensive care plans based on current best practices.
• Demonstrate wound care techniques and provide education materials.
• Educate patients and families to support learning and engagement.
