Job summary

Northfield Hospital + Clinics in Minnesota is seeking an experienced RN Case Manager - Wound Healing to deliver direct nursing care, assess patients, and craft care plans using evidence‑based practice. You will collaborate with physicians and the care team to optimize outcomes for medical/surgical patients.

The role is a 0.6 FTE schedule (48 hours per pay period) with a daytime shift from 7:50 AM to 4:30 PM. A MN RN license, BLS, CPI and wound care experience are preferred.

Qualifications

• Current RN license with the Minnesota Board of Nursing, in good standing.
• BLS enrolled in NH+C RQI program and complete requirements within 90 days of enrollment.
• CPI certification within six months.
• Workplace Violence education in New Employee Orientation and annually thereafter.
• Trauma training meeting trauma system criteria within 1 year of hire.
• EKG rhythm interpretation competence documented in first 6 months.
• Professional appearance and demeanor.
• Effective communication skills, both verbal and written English.
• Minimum one-year current wound care experience preferred.

Responsibilities

• Assesses patient’s condition based on observation, examination, interview, labs and other data.
• Completes assessment in specified time frame.
• Identifies changes in normal and abnormal parameters in patient’s condition.
• Uses professional judgement to prepare, prioritize and execute the patient’s plan of care.
• Directs and delegates staff in implementing patient’s plan of care and provides supervision.
• Implements nursing interventions to achieve progress toward outcomes or to stabilize condition.
• Administers medications with BMV scores >95%, starts IV fluids and blood products.
• Performs IV venipuncture, restraint application, sterile dressings, etc.
• Evaluates and documents patient response and updates treatment plans.
• Provides patient/family education based on diagnosis and treatment plan.
