Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate agency-wide programs for prevention and management of skin breakdown. You will provide expert consultation and hands-on care for patients with acute or chronic wounds and establish multidisciplinary care plans.

The role features leading educational programs for staff and patients, driving quality improvement initiatives, and coordinating regional wound care metrics to ensure high-quality outcomes.

Qualifications

• RN required and WOCN certification within 6 months of hire.
• Bachelor’s Degree in Nursing required; MSN preferred.
• Minimum two years nursing experience in a clinical specialty area; two years in homecare preferred.
• Strong written and verbal communication skills.
• Ability to communicate at all levels within the organization and lead planning for patient outcomes.
• Knowledge of wound care standards and ability to balance cost, quality, and patient outcomes.
• Proficient with MS Outlook, Word, Excel, PowerPoint, and MS Project.

Responsibilities

• Develops and coordinates agency-wide wound care programs for prevention and management of skin breakdown.
• Provides expert consultation and hands-on care for patients with acute or chronic wounds.
• Establishes and coordinates multidisciplinary, comprehensive management plans.
• Coordinates care for populations of patients and develops standardized protocols.
• Develops educational programs for patients, staff and other health care providers.
• Drives quality improvement initiatives to optimize wound outcomes.
• Advocates for patients with skin and wound care needs.
• Reviews regional wound program performance metrics with leadership.
