Job summary

Twin Cities Physicians is seeking a full-time Wound Care Nurse Practitioner to deliver advanced wound care within assisted living and memory care communities across the Twin Cities Metro. You will lead assessments, plan treatment, and partner with a regional care team.

This on-site position offers a compensation range of $120,000 to $160,000+ total compensation, mileage reimbursement, and a comprehensive package including PTO, CME allowance, malpractice coverage, and full benefits.

Qualifications

• Active Minnesota CNP license.
• Experience in wound care, geriatrics, primary/post-acute care preferred.
• Ability to travel within Twin Cities Metro.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management within senior living communities.
• Conduct routine rounding visits and follow-up care across assigned communities.
• Manage medications, diagnostics, treatment plans, and interventions.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Identify risks early and implement strategies to prevent complications and hospitalizations.
• Support continuity of care while improving healing and quality of life.
