Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner (AGNP/CNP) to join a collaborative team serving assisted living, memory care, skilled nursing, and long-term care communities throughout Minnesota.

This full-time role offers $120,000 to $160,000+ total compensation, mileage reimbursement, full benefits package, PTO, CME allowance, malpractice coverage, and DEA reimbursement to support high-quality patient outcomes.

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Must be able to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care services, treatment planning, and management of complex wounds.
• Conduct rounding visits across assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Support preventative care initiatives and continuity of care.

What You'll Be Doing

• Provide advanced wound care services, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate closely with physicians, nurses, therapists, caregivers, and facility leadership.
• Support preventative care initiatives and continuity of care efforts.
• Help improve healing outcomes while reducing avoidable complications and hospitalizations.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
