Job summary

Twin Cities Physicians seeks a Nurse Practitioner to provide advanced wound care and geriatric rounding inside assisted living and skilled nursing facilities across the Twin Cities Metro.

The role is full-time with compensation of $120,000 to $160,000+ total, plus mileage reimbursement and a comprehensive benefits package including CME support, malpractice coverage, DEA reimbursement, PTO, and more.

Qualifications

• Active Minnesota CNP license; AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and ability to manage medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing wound management.
• Conduct routine rounding visits within assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support chronic disease management and continuity of care initiatives.
• Help improve healing outcomes while reducing avoidable complications and hospitalizations.
